This form is used explicitly to allow The Health Trust to notify your new provider of your personalized meal and grocery delivery instructions. Below, you will find the ROIs available to printout. Only one ROI is needed but we are sharing these in a variety of languages to best assist you or your family member.
1. Digitally return your ROI – Take a photo of your completed ROI with your phone (or scan with your home printer) and email to: MOW@healthtrust.org
2. Hand your ROI directly to your MOW/MTM delivery driver upon the next visit.
