Eight Organizations Receive Strategic Investments to Improve Food Access, Strengthen Supportive Housing, and Protect Access to Healthcare
San Jose, CA — The Health Trust today announced $2.15 million in grant funding as part of its first cycle of Fiscal Year 2026 grantmaking, supporting eight community-led initiatives that address some of the region’s most urgent health challenges. These awards reflect The Health Trust’s mission to advance health equity across Santa Clara and Northern San Benito Counties by funding organizations focused on food access, chronic disease prevention and management, and supportive housing.

The FY26 Cycle 1 awards include seven Health Partnership Grants and one Health Equity Fund Grant, with projects ranging from culturally tailored wellness programs to large-scale healthcare system support and legal advocacy to increase access to public benefits for underserved populations.
FY26 Cycle 1 Grantmaking Highlights
Food & Nutrition
- Fresh Approach is developing and implementing the data, billing, and referral infrastructure needed to deliver CalAIM Community Support services to Medi-Cal patients in Santa Clara County.
- Korean American Community Services is strengthening its capacity to serve the Korean and broader AAPI community through culturally responsive food and wellness programming.
- West Valley Community Services is improving self-sufficiency and better health outcomes through its Community Access to Resources and Education (CARE) program, combining food distribution, case management, and referrals for low-income families.
Chronic Conditions
- Community Health Partnership, in collaboration with six Federally Qualified Health Centers, is increasing Medi-Cal enrollment and retention among low-income residents while improving access to culturally and linguistically appropriate preventive care.
- Valley Health Foundation is strengthening the public healthcare system across Santa Clara County.
Supportive Housing
- Amigos de Guadalupe is fortifying services for unhoused families, helping them secure and maintain permanent housing.
- Family Supportive Housing is enhancing its Shelter and AfterCare programs that support the health and long-term stability of families experiencing homelessness.
Health Equity Fund
- Law Foundation of Silicon Valley is providing free legal services that ensure access to public benefits, healthcare, and housing for low-income individuals.
