The Health Trust Awards Nearly $400,000 in Grants to Address Health Disparities in San Jose, Gilroy, and Northern San Benito County

San Jose, CA  – On March 22, 2023, the Health Trust’s Board of Trustees approved four new grants totaling $391,568. The grants are part of quarter three of the Health Trust’s fiscal year 2022-2023 $2.3 million grantmaking budget, supporting community organizations and public agencies aligned with The Health Trust’s mission of building health equity in Silicon Valley. The four Health Partnership Fund grants have been awarded to Family Supportive Housing, Jovenes de Antaño, St. Joseph’s Family Center, and YMCA of San Benito County.

Together, the approved grants aim to improve health in our community by integrating wellness services into housing; addressing food insecurity for older adults; strengthening nonprofit organizational capacity; and supporting new partnerships between clinics and community-based health programs. 

Health Partnership Grant Awards

Health Partnership Grants support organizations, projects, and programs that contribute and align with our mission and three focus areas: Improving Health Through Food, Making Chronic Conditions More Preventable & Manageable, and Prioritizing Health In Housing.

Family Supportive Housing

To provide families experiencing homelessness in San Jose the resources, support, and access to healthcare needed in order to make healthy lifestyle choices to promote self-sufficiency and wellness through their Healthy Family Program.

Jovenes de Antaño

To improve the health and well-being of San Benito County seniors who are homebound and low-income by providing daily home delivered hot meals and welfare checks.

St. Joseph’s Family Center

To support the long-term sustainability of St. Joseph Family Center, a safety-net service organization in Gilroy, as it embarks on an organizational and leadership restructure. 

YMCA of San Benito County

To pilot the Healthy Family Home program: a 12-month program designed to help San Benito County families learn and create healthy habits together, lowering their risk of developing Type-2 diabetes through long-term family lifestyle changes.
