The Health Trust Awards Over $400,000 to Organizations Addressing Health and Nutrition Needs in Gilroy and East San Jose

San Jose, CA – On March 24, 2021, The Health Trust’s Board of Trustees approved three new Health Partnership grants totaling $431,700 to organizations serving under resourced communities in East Jose and South Santa Clara County. The three grants are part of the third quarter of The Health Trust’s $2.2 million annual Health Partnership Fund allocation supporting community organizations and public agencies aligned with the organization’s mission of building health equity in Silicon Valley.

The grants have been awarded to Gardner Health Services, St. Joseph’s Family Center, and The Foundation for Hispanic Education to address health and nutrition needs in the community.

Together, the approved grants will strengthen food ecosystems, organizational capacity, and infrastructure to increase access to healthy foods for vulnerable residents, and remove barriers to access to care for patients with diabetes. 

In the first three quarters of fiscal year 2021, The Health Trust has invested $2,893,830 towards 18 organizations – 15 of those grants have been funded through the Health Partnership Fund, and three grants have been funded through a temporary $1M increased payout from the foundation’s endowment in support of communities disproportionately impacted by COVID-19.  

Health Partnership Grant Awards

Gardner Health Services
To support full access and quality of care in disease management to patients with diabetes at its Gilroy clinic. Gardner plans to provide its patients with the required case management resources to help improve their health outcomes, including a reduction of HbA1c levels and improved ability to self-manage their diabetes.

St. Joseph’s Family Center
To support essential safety-net services in order to meet the growing need for food assistance in South County and strengthen the resiliency and capacity of SJFC’s services through staffing and infrastructure. 

The Foundation for Hispanic Education
To strengthen TFHE’s infrastructure and capacity through the establishment of a state-of-the-art kitchen, a critical component of the Finn Center Multi-Purpose Building renovation, as a part of the Healthy East Side Initiative. As a result, TFHE will address food scarcity and malnourishment by increasing healthy food access for its students, families, and the East San Jose community.
