The Health Trust Makes Grants Totaling over $1.3M

San Jose, CA – On Wednesday, June 26, 2019, The Health Trust Board of Trustees approved seven grants totaling $1,323,389 to support The Health Trust’s mission of building health equity in Silicon Valley. Some of the approved grants will: support the creation of affordable housing sites that are service enriched for people who require Long Term Services & Support (LTSS); develop a referral program to community-based chronic disease self-management programs for high need patients; reduce STI and HIV infections in Santa Clara County by expanding and institutionalizing multisite testing; and support projects that encourage the most vulnerable, hard-to-count populations in Santa Clara County to participate in the 2020 census.

Breakdown of Grants

Asian Americans for Community Involvement (AACI)
Over the next 12 months, AACI looks to institutionalize a referral program that will refer patients who are at high risk of developing and those currently experiencing a chronic condition, to community-based chronic disease self-management programs. In an effort to ensure that patients are accessing self-management programs, AACI seeks to establish a strong referral network, one that will be used by patient navigators who will provide a warm hand-off from patient to community partner.

Community Health Partnership
Over the next 18 months, Community Health Partnership will convene regional partners, clinic members, and patients to co-design, plan, and test solutions that address social determinants of health issues for patients with diabetes using a “human-centered approach” model.

Planned Parenthood Mar Monte (PPMM)
Over the next 12 months, PPMM seeks to reduce STI and HIV infection rates in Santa Clara County (SCC) by expanding and institutionalizing multisite testing at its six SCC sites. Multisite testing refers to testing the three parts of the anatomy most likely to be infected – vagina, throat, rectum.

Public Health Institute
To advance The Health Trust’s mission of building health equity in Silicon Valley, the Public Health Institute will support the Public Health Professional Development and Management Program through the placement of fellows who will work on community health issues in areas of food, chronic conditions, and health in housing.

Silicon Valley Community Foundation
To ensure a fair, accurate census count in 2020, this grant will support the Santa Clara County Pooled Funds that will be used to award grants to nonprofit organizations for projects that encourage the most vulnerable, hard-to-count populations to participate in the census.

Working Partnerships USA
Over the next 12 months, Working Partnerships seeks to continue its work to ensure that the LTSS Integration Committee creates service enriched housing opportunities in Santa Clara County for people who need LTSS services and address system flaws and policy gaps that impede the provision of additional housing options and related services. To achieve this, the Committee will complete models for types of services and financing of services at two LTSS housing projects currently in development.

Youth Alliance
Through this grant, Youth Alliance will strengthen its Community Health Project that will broaden the availability and expanded levels of behavioral health services for underserved/unserved youth in San Benito County.
