The Health Trust makes a Health Partnership Grant totaling $75,000

San Jose, CA – On December 12, 2018, The Health Trust Board of Trustees approved one Health Partnership Grant totaling $75,000 to build health equity in Silicon Valley.

Indian Health Center of Santa Clara Valley
Aligning with our focus area of Making Chronic Diseases More Preventable and Manageable, the Indian Heath Center has been granted $75,000 over 9 months to update and improve its policies, procedures, and workflows to implement an opt-out HIV screening technique, expand PrEP/PEP services, and develop a referral process that will ensure that patients diagnosed with HIV are connected to care.

As of 2017, there were 3,361 people living with a diagnosis of HIV/AIDS in the County of Santa Clara, with 156 new cases in 2017. The State of California estimates that another 9% of persons with HIV are unaware of their infection. In Santa Clara County, Latino/Hispanic and Black/African American populations are heavily impacted by the HIV epidemic. Because the Indian Health Center works with a large Latino population, they seek to increase Guideline-based STI/HIV screening rates, expand its PrEP/PEP services, and develop a referral process that will ensure that patients diagnosed with HIV are connected to care. IHC plans to ensure that this work is institutionalized into its clinics and as such they plan to update and improve its policies, procedures, and workflows, and increase testing by 10% in the first nine months.
