After a personal tragedy late in his life, Gary lost everything and became homeless – living out of his car for three years. Things got worse including hospitalization for a hip injury, malnutrition, and several infections. The doctors weren’t sure he would even recover.

Enter The Health Trust’s intensive case management services for chronically homeless. After his yearlong recovery, we coordinated Gary’s care with a 180-day goal plan to start turning his life around. The comprehensive services include securing housing and income, medical and counseling support, and access to county resources.
