Food is Medicine makes change to eligibility requirements

The Food is Medicine Coalition is excited to announce that the Medi-Cal Medically Tailored Meals Pilot Program can now be offered to more clients, as eligibility requirements have changed. Prospective clients no longer need to have been recently discharged from a healthcare facility to be eligible. Moving forward, meals can also be provided to anyone who has congestive heart failure, and has been to the hospital, emergency department, or to a skilled nursing facility in the last 12 months due to exacerbations of heart failure. Other eligibility requirements still apply, including being on Medi-Cal for the last 12 consecutive months and having seen a primary care physician or specialist at least once in the last 12 months.
