Director of Healthy Aging
The Healthy Aging Director has primary responsibility for and will be pivotal in providing leadership and expertise to drive policy and advocacy efforts that promote the Health Trust’s Aging agenda; designing a grantmaking strategy with the Program Officer that ensures Health Trust Aging grants are aligned with Healthy Aging objectives; managing Healthy Aging programs and staff; and developing strategic partnerships that will garner support for Healthy Aging strategies.

The Community Worker will provide promotion, prevention, early intervention information, resources and support to families with children prenatal through age five through a wide range of activities/services focusing on school readiness, health and early literacy.

Benefit Specialist
Assess the needs of individuals with HIV/AIDS, and connect them with internal services and/or community resources; assist clients with accessing and/or troubleshooting public/private benefits and entitlement programs; coordinate with staff and other providers (HIV and non-HIV related) to attempt to get client needs met and to avoid duplication of services; outreach to clients in jail or who are out of care, outreach to the community.

The Project START Lead provides direct social work and case management services to clients with HIV in the County of Santa Clara Correctional Facility to provide prevention education and services in efforts to prevent further spread of communicable diseases, stabilize health and well – being, promote permanent and stable housing, improve quality of life and avoid costly institutional care.

Provide intensive medical case management services and support to a caseload of 20 chronically-homeless community members in order to improve clients’ quality of life.

Housing Case Management Specialist
The Housing Case Management specialist is responsible for managing a caseload comprised of clients receiving assistance through the Housing Subsidy Program. The position performs complex, advanced technical and administrative tasks and assists clients in the initial and annual application process; conducts detailed interviews to obtain complete and accurate information; develops and accumulates available housing resources and implements the processes necessary to assist clients secure and maintain stable, safe, and affordable housing and related services in Santa Clara County.
