Overview
Over the last 10 years, The Health Trust has provided over $100 million of community benefit
through program services and grantmaking activity. Our programs include Meals on Wheels, AIDS
Services, Children's Dental Services, Community Health Promotions, Family Health Insurance,
Community Partnerships and Administrative Support Services. The Health Trust is a 501c3 community
benefit organization.
Services of The Health Trust
AIDS Services
800+ HIV/AIDS clients served annually
The Health Trust operates the largest non-medical program in Santa Clara County for individuals and families
living with HIV/AIDS. Services include: nursing and social work case management; emergency financial
and housing assistance; food, including home delivered meals; home care; a resource center; and AIDS education.
Family Resource Centers
Funded by FIRST 5 Santa Clara County’s Learning Together Initiative, The Health Trust’s Family Resource Centers provide free services and activities for families with children ages 0-5 that promote school readiness, strengthen families and encourage healthy eating and physical activity.
Financial and Administrative Support Services

The Health Trust shares its financial management expertise by providing back office accounting support,
at or below cost, to many of the Silicon Valley’s most notable nonprofits allowing
them to focus their time and resources on delivering services to the community.
Meals On Wheels
97,000 meals delivered annually
Respect, dignity and the ability to maintain their independence in their own home are the goals
of seniors and adults with disabilities or serious health issues. Meals On Wheels brings nutritious, hot meals to their door,
along with wellness checks, disaster packs and a friendly smile.
Oral Health and Dental Services
15,000 children receive dental care annually
The Health Trust supports the Children's Dental Center in East San Jose that offers widely accessible, quality dental care in a central location for residents. The Health Trust is also leading a movement to fluoridate all of San Jose.
Wellness Access and Education
This service line provides critical support and education to our community's most underserved members. By providing culturally competent health insurance enrollment, health care navigation, and prevention and self-management programs, we help to improve health outcomes and reduce disparities within many populations. We serve thousands of individuals each year and have a strong presence throughout the most underserved areas in the County.
