Wellness Programs

The Health Trust offers a variety of wellness programs aimed to help individuals and older adults lead more healthful and independent lives. Our programs are evidence-based and led by a team of trained instructors (scroll down to meet them!) who can provide important health resources. You can inquire about enrollment for yourself or to help care for an ailing friend or loved one.

Better Choices, Better Health

Better Choices, Better Health: A six-week self-management program for people living with with chronic conditions such as high cholesterol, arthritis, diabetes, asthma, depression, obesity among others. This program also benefits individuals who care for someone living with chronic conditions. Workshops are fun, interactive, and designed to enhance regular treatment and disease-specific education.Offered in English, Spanish, Vietnamese, Cantonese, Mandarin. Learn about:

  • Techniques to deal with problems such as frustration, fatigue, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • How to evaluate new treatments

Diabetes Self-Management Program

Diabetes Self-Management Program: A 6-week self-management program for people living with diabetes. This program also benefits individuals who care for someone living with diabetes. The workshop is currently offered in English and Spanish. Learn about:

  • Techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration
  • Appropriate exercise for maintaining and improving strength and endurance
  • Healthy eating
  • Appropriate use of medication
  • Working more effectively with healthcare providers

A Matter of Balance

A Matter of Balance: an 8-session award-winning program designed to manage falls and increase activity levels. Offered in English and Spanish. Learn about:

  • exercise to increase strength and balance
  • make changes to reduce fall risk at home
  • set goals for increasing activity
  • and so much more

  • The cost of these workshops may be covered by your health insurance. We deal directly with your insurance. You do not have to worry about billing.
  • Some workshops are sponsored by organizations and are free to participants that fit certain criteria.
  • We have some funds available for scholarships.
  • Currently, online registration is only available for BCBH and DSMP. Find the forms under each program menu.

Locations

These workshops are offered throughout six counties, which include: Santa Clara, San Benito, Santa Cruz, San Francisco, Alameda, and San Mateo County at collaborating community-based organizations including senior centers, low-income housing, public libraries, and many others.
