The Health Trust provides a variety of direct services aimed at improving the health and wellbeing of vulnerable populations in Santa Clara and northern San Benito counties. We care for people with HIV/AIDS, help older adults maintain their health and independence, assist families in obtaining health insurance, care for children’s teeth and provide opportunities for people of all ages to eat well and be active.

AIDS Services
2013-12-11 12.50.37The Health Trust operates the largest non-medical program in Santa Clara County for individuals and families living with HIV/AIDS, serving more than 800 clients annually. Services include: nursing and social work case management; emergency financial and housing assistance; food, including home delivered meals; home care; a resource center; and AIDS education.

Better Choices, Better Health/Diabetes Self-Management
People living with chronic conditions or diabetes can learn to manage their conditions to live healthier, more independent lives. Better Choices Better Health is a six-session, peer-taught health education workshop that helps individuals take day-to-day responsibility for their care and to increase their skills needed to manage their disease and work effectively with their health care professionals.

Children’s Dental Services and Oral Health Education
The Health Trust provides oral health outreach and education through promotoras, Health Educators and Lay Health Workers who are community health workers trained on general concepts about oral health. The promotoras conduct outreach in English and in Spanish and recruit families to host home meetings where the oral health education is delivered to small groups of children and adults. Emphasis is placed on four principles of oral health: proper brushing, daily flossing, balanced diet and seeing the dentist on a regular basis. Last year, over 1,000 individuals received oral health education in homes and community sites. The Health TrustThe Health Trust, in partnership with Children’s Dental Group, has Children’s Dental Centers in East San Jose and Sunnyvale. The Children’s Dental Centers represent an innovative approach to providing pediatric dentistry that is significantly improving access to high-quality dental care for the children of Silicon Valley. It combines the latest equipment, technologies and best practices of pediatric dentistry with a fun, kid-friendly environment that is welcoming to all children and families.

Health Insurance Enrollment
OAHFalfonzoThe Health Trust provides access to health care for uninsured adults and children in Santa Clara County by enrolling them in available low-cost insurance programs including Covered California, MediCal and Healthy Kids. Our Enrollment Specialists are bicultural and bilingual and are trained to be culturally sensitive and proficient.

Learning Together Initiative/Family Resources Centers
The Health TrustFIRST 5 Santa Clara County’s Learning Together Initiative funds The Health Trust’s Family Resource Centers, which provide an environment where children ages 0-5 and their parents and caregivers can learn and grow together. These Family Resource Centers, operated in partnership with Catholic Charities of Santa Clara County and SJB Child Development Centers, offer free programs and services designed to strengthen families and promote literacy, proper nutrition, physical activity and skills that growing children need to be ready to succeed in kindergarten.

The Health Market

These include health insurance enrollment, oral health education, nutrition and wellness education, as well as chronic disease prevention and management.
